Physical Exambimanual exam

Many fibroids are found at the time of a bimanual pelvic exam. During this exam your provider will place one hand on the abdomen and simultaneously place 1-2 fingers in the vagina. The uterus can then be felt between the two hands and the uterine shape and size can be assessed. If the uterus is enlarged or irregularly shaped then fibroids may be present. If the doctor thinks you may have fibroids she or he will likely order a pelvic ultrasound for further evaluation.

Ultrasound transvaginal ultrasound

Ultrasound is a common test that can visualize the pelvic organs well. This test is often performed both abdominally and trans vaginally. This test is not generally painful and the results can be determined almost immediately. Sound waves are used to create a picture of the uterus and ovaries. There is no radiation used.

This exam generally takes about 15-30 minutes to perform. You will usually be asked to have a full bladder during the exam. The exam begins abdominally with the probe being placed on the abdomen. The second portion of the exam is done trans vaginally and is not painful. The probe is placed in the vagina and provides excellent close up pictures of the uterus and ovaries.

Your provider will review the results with you after the exam

Sonohysterography

sonohystogramThis is another type of ultrasound procedure with gives superb visualization of the interior of the uterine cavity. During a trans vaginal ultrasound, sterile saline is gently inserted through the cervix via a small catheter. The saline expands the uterine cavity and makes it possible to look for fibroids and/or polyps inside the uterus. Many women experience some short-lived cramping (like menstrual cramping) during the procedure. This usually stops right after the procedure is over.

Magnetic Resonance Imaging (MRI)

Magnetic Resonance ImagingRarely, more exacting visualization is needed to evaluate the uterus. In these situations a magnetic resonance imaging (MRI) may be indicated. This is often necessary if uterine artery embolization (UAE) is being considered. This test takes about 45 minutes. During this time you will need to remain still. An MRI is not painful and there is no radiation used.

Hysteroscopy

HysteroscopyHysteroscopy is a minimally invasive procedure performed in the office or hospital, which allows the surgeon to look directly into the uterine cavity. In addition to looking into the cavity, the surgeon can also removed polyps and fibroids. During this procedure a small camera is passed through the cervix and into the uterus. Hysteroscopy generally takes about 30 minutes to perform.
